Patient Selection:

1. Labia Majora with a great deal of fat

   

   

   

   

2. Outlining in her clothing

   

3. Chronic irritation, sweating, discomfort

4. Here are my thoughts on the procedure:

   1. Do these in the surgery center until your comfort level is higher. Don’t do these Fat Pad Reductions unless you have the basic Majoraplasty skills down pat.

   2. It is hard to control pain. Fat has lots of nerves. You will need about 20cc of local per side. Probably more.

   3. This can be bloody.

   4. You may need to place multiple interrupted 4-0 Vicryl sutures to control the bleeding

   5. Standard cautery and Bipolar RF work well to control bleeding

   6. CO2 Lasers are not very effective in controlling the bleeding

   7. Removing too much majora skin can cause a gaping appearance.
